About the Supplementary Income Payment (Updated April 1st, 2009)
 1. Objective of the Supplementary Income Payment

Designed for those coping under stresses of the current economic recession, the Supplementary Income Payment (Teigaku Kyufu-kin) is distributed as a stimulus for regional economies via livelihood support and widespread provisions to residents.

  2. Qualified Recipients and Authorized Payee Information

<A> Those qualified for the Supplementary Income Payment must satisfy conditions (1) or (2) as of the Standard Date (February 1st, 2009):

( 1 ) Persons registered as inhabitants of KokaCity in the basic citizen registry.
( 2 ) Persons registered in KokaCity’s index of foreign residents (excludes Temporary Visitors [tanki taizai] and illegal residents)

<B> Authorized Payees shall be the head of each household. However, Foreign Residents must apply for receipt of the Supplementary Income Payment individually.

Foreign Registrants whose stay period expires between April 15th, 2009 and the Payee Decision Date (kofu kettei-bi) (about 1 month after applying) must renew their stay period at their regional immigration office (chiho nyukoku kanri-kyoku) and update their registration at their municipality of residence.

 3. About the Payee Amount

12,000 yenper qualified person(s)

20,000 yenper qualified person(s) aged 65 and over (born before 2/02/1944); or aged 18 and under (born after 2/02/1990)

 4. Application/Payment Process

(Note: Foreign Residents must apply individually, rather than via head of household.)

(1) Application Process
①Please check and confirm the contents of the Supplementary Income Payment Application/Invoice envelope (labeled 定額(ていがく)給付(きゅうふ)金(きん)申請書(しんせいしょ), regarding payment recipients and payment amounts).

Please use the “Entry Guide” on the back of the blank application form as a reference for filling out and stamping all necessary items (with your stamp/inkan).

Please append on the back of the application documentation necessary for identity verification (*1) and wired transfer bank account verification information (*2).

Please refer to items *1 and *2 on the back of the application.

③ You can apply either via post or city office:

A)Application via post・・・
Please submit via mail both the ①application and necessary documentation※ (② copies of identity verification documentation, and ③ bank account ledger) in the reply envelope included in the application packet. After confirming your documentation, payment will be transferred electronically to your designated bank account. We will notify you of the transfer date in advance.

Foreign Residents must submit a copy of both sides of their Foreign Registration Card (Gaikokujin Toroku Shomei-sho)

B)Application via city office・・・
Proceed to city hall to apply through this method. Necessary documentation and distribution occurs the same as in (A) above.

(2) Payment Receipt Process

There are two methods for payment distribution.

1. Wired Bank Transfer・・・Wired bank transfer (furikomi) to the recipient’s bank account is the default.

2. Cash Receipt via City Office・・・Cash receipt via city office (madoguchi) is for those who possess no account at a financial institution, whose bank accounts have been suspended, or who have difficulty receiving payment via bank transfer.

The procedure for application is as follows: ①Submit application to a city reception window (madoguchi)⇒②Receive a city notice of exchange date⇒③Bring exchange notice to corresponding reception window at city hall⇒④Receive payment via cash.

How to Apply via Proxy/Representative
A representative can apply in lieu for the following applicants (target payees):

Persons who are members of the applicant/recipient’s family

As of the standard date, foreign persons who share the same address as, and are living with the applicant/recipient (the target payee)

Legal representatives of the applicant/recipient (persons exercising parental
authority, minor or adult legal guardians, guardians or aids bestowed legal representation status)

Persons recognized by the mayor as District Welfare Officers, Neighborhood Council Chairs, and relatives who take regular personal care of the applicant/recipient

(Note)Even when applying via proxy, enter the original applicant’s name.

In the Event the Applicant/Recipient (the target payee) Dies or Passes Away
1. 【Persons registered in the Basic Citizen Registry】In the event of the death of the applicant/recipient on or after the Standard Date, persons who correspond with the new applicant/recipient must apply. If the household dissolves as a result of this difficult situation, a representative selected from the corresponding household must apply.

2. 【Persons registered in the index of foreign residents】In the event of target payee death on or after the Standard Date, a representative registered in the basic citizen registry or index of foreign residents, who shared the same address and lived with the target payee, must apply for the Supplementary Income Payment.

   Reception at the Minakuchi city office also available on
   4/18, 4/19, 4/25, 4/26, and 4/29.

Applications not received by the deadline are considered invalid for payment receipt.

 6. Payout Start Date

Wired bank transfers are planned to occur 3 times per month (beginning, middle, end).

Because we must confirm various documentation (designated bank account information) and determine allocations after receiving your application, transfers will not be received until about after 2 or 3 weeks (1 month for Japan Postal Bank Accounts). As of now, the first set of wired bank transfers are planned for the end of April.

If receiving payment via city office reception window, please understand it will take about 3 weeks after application submission, and the start date will occur later than wired transfers.

 7. Other Important Notes

(1) In the event payment cannot be transferred due faulty transfers as a result of inadequate application, if important corrections are not made on top of confirming appropriate information with the city, the application will be recognized as being withdrawn.

(2) If due to false information the Supplementary Income Payment is obtained by incorrect means, you will be asked to return the distributed Supplementary Income Payment.

(3) You do not bear the right to distribute the Supplementary Income Payment or present it as collateral.

(4) In the event there is anything unclear about your application, the city will make an inquiry with you. However, we will absolutely not ask you to make use of your ATM (Automated Telling Machine) or ask that you transfer any handling fees. If you receive an unidentified phone call, please contact the city reception office or police department immediately.
